The Answers You Need

HOW DO I SUBMIT AN ORDER FOR SERVICE?

For Individuals:

1.) Fax / Email Providers orders (Lab requisition) to (402) 500-7975 or email  orders to phleblifelabs@phleblifellc.sprucecare.com.

2.)You may also want to contact your provider and have them send over an updated order for any labs / services that you may need done if you do not already have one. This can speed up the process in receiving your services.

3.) Contact us at (402) 534-1186 to assure orders were received.

** NOTE: IF YOU ARE A NEW PATIENT TO US, PLEASE DO NOT USE THE ONLINE SCHEDULING OR PREPAYMENT OPTIONS. PLEASE CONTACT OUR OFFICE AT (402) 534-1186 TO COMPLETE YOUR NEW PATIENT INTAKE AND SCHEDULE YOUR FIRST APPOINTMENT.

For Providers / Facilities:

1.) Submit an order via our "Submit Order" tab located on Our Services page.

2.) Print out our lab requisition form, completely fill it out, and fax over to (402) 500-7975 or email to phleblifelabs@phleblifellc.sprucecare.com. Please provide your patient with the original lab requisition and the current lab that your facility uses to assure proper transportation and processing of specimens.

3.) Please be sure to include on the order where to fax lab results to as well as front and back of patients insurance card(s) and demographic / face sheet.

***Any information that is missing may delay the processing of specimens***

4.) We will contact the patient to schedule an appointment at their home or business if they have not scheduled an appointment via our website.

​

5.) A confirmation of the scheduled appointment date and time will be sent to both patient and provider via preferred method of contact.

​

6.) All completed and processed labs will be sent to the designated lab.

​

7.) All test results are sent directly to the ordering provider from the designated lab.

​

8.) All other services that has been ordered by the patients provider will be sent directly to the provider via preferred method of contact.

I RECEIVED AN EMAIL CONFIRMATION THAT SAYS, "LOCATION VARIES". HOW DO I KNOW WHERE I WILL GET MY SERVICES DONE?

Phleb Life LLC services many patients and providers. Due to this, the location site may sometimes vary. When we complete a verbal conversation confirming a patients appointment date and time, the location of the draw will be confirmed as well.

HOW MUCH NOTICE DOES SERVICE REQUIRE?

For next day service, all orders must be  submitted by 3pm the day before collection. Any orders not received prior to day of collection, may result in a cancellation of services on day of collection and patient to be rescheduled. Any patients that need same day service, please call to check availability. 

DO YOU ACCEPT PT/INR ORDERS?

Yes. All orders must be sent prior to collection date. For all standing orders, patient may schedule via our website under "capillary draws"

WHEN WILL I BE SCHEDULED FOR MY APPOINTMENT?

You or your provider may schedule right away via our website. Otherwise, you may call us to schedule an appointment. All providers orders must be sent by 3 pm the day before collection. If this is a same day service, orders must be sent with the patient or immediately faxed to (402) 500-7975 or via email at phleblifelabs@phleblifellc.sprucecare.com. Any orders not received in a timely fashion, may result in a cancellation of services and reschedule at a more convenient time.

HOW LONG SHOULD I FAST FOR MY FASTING LABS?

For fasting lab draws, we only require a patient to be fasting for 8 - 10 hours. Please advise us if you have been fasting longer than the required amount of time as this may affect the patient at the time of draw.

ARE SERVICES PROVIDED AFTER 3PM?

Our laboratory kits and tests has set requirements for processing. Due to this, we highly recommend all lab tests are performed prior to 3pm to decrease any lab or processing errors. However, we DO provide services after 3pm. You or your provider must call to arrange any services after 3pm.

DO YOU PROVIDE NEXT-DAY SERVICE?

Yes. All next day request received prior to 3pm will be placed on the collection schedule for the next day as long as there is availability to accommodate . Any request after 3pm will be placed on the collection schedule following the next available business day. Incomplete or inaccurate orders will require extra time and communication and may not make the schedule for the following business day.

DO YOU PROVIDE SAME-DAY OR STAT SERVICE?

At this time, Phleb Life LLC does not provide any same-day or STAT service. 

IS THERE AN EXTRA CHARGE FOR SAME-DAY, STAT, OR WEEKEND SERVICES?

Typically, there is an additional $15 charge for same-day service or same-day STAT service. There is an additional $20 charge for weekend services. However, at this time, we do NOT provide any same - day, STAT or weekend services.

ARE WEEKEND SERVICES PROVIDED?

Our normal business hours are Monday through Fridays from 9am to 3pm. Weekend services can be provided. However, you or your provider must call by Friday at 3pm to arrange these services. All lab orders, demo / face sheet and copy of patients insurance card (front and back) must be received by 3pm. ***Note: There is an additional $60 charge for weekend services***

ARE SERVICES PROVIDED FOR SPECIALTY KITS COLLECTION ON THE WEEKENDS?

No. Due to the shipping services our courier service provides, specialty kits collection can only be performed Monday through Friday by 3pm.

IF I MISSED THE 3PM ORDER CUT OFF PERIOD ON FRIDAY, CAN I STILL RECEIVE SERVICES FOR MONDAY?

Phleb Life LLC does its best to accommodate our patients and providers that we provide services for. Please call us to check availability and we may be able to coordinate services for Monday dependent upon the collection schedule. If we are unable to get you scheduled for Monday, we will place you on our collection schedule for Tuesday.

HOW DOES THE PATIENT / PROVIDER KNOW THAT PHLEB LIFE LLC IS COMING TO COLLECT LABS?

After the patient has been placed on the collection schedule, Phleb Life LLC will contact the patient or patients living facility the night before the collection to coordinate our phlebotomy services visit. If the patient resides at a living facility that Phleb Life has already prearranged scheduled lab draws, the facility will reach out to Phleb Life to inform of the scheduled lab draw by 3pm the day prior to the scheduled lab day.

WILL PHLEB LIFE LLC ATTEMPT COLLECTION WITHOUT FIRST RECEIVING PATIENT OR PATIENTS FACILITY CONFIRMATION?

Phleb Life LLC will make every attempt to contact the patient or patients facility to coordinate the visit the day before services are rendered. Phleb Life LLC will not show if a verbal confirmation has not been received the day prior to the collection date.

WHAT HAPPENS IF PHLEB LIFE LLC IS UNABLE TO COLLECT A SPECIMEN ON A PATIENT FOR A PREARRANGED SERVICE DUE TO A SPECIFIED REASON?

Phleb Life LLC understands that situations come up. Phleb Life LLC will always try to work to get a patient rescheduled at their convenience. However, if a patient failed to fast for fasting labs, failed to drink plenty of water for the draw, patient refused lab draw, patient is not scheduled or any other specified reason, the patient will remain responsible for the $25 charge. This also is based on a case by case basis and will be up to the discretion of Phleb Life LLC whether or not a patient will be charged.

IS PROCESSING AND HANDLING DONE BY PHLEB LIFE LLC?

Yes. Phleb Life LLC will handle and process all specimens per your designated laboratory's specimen requirement. All specimens requiring time to sit will be allowed time to sit for 30 minutes, centrifuged, aliquoted (as needed), refrigerated or frozen under the protocol required.

DOES PHLEB LIFE LLC SHIP OR DELIVER COLLECTED SPECIMENS TO A PREFERRED LABORATORY?

Phleb Life LLC works directly with Boston Heart and Veridia Diagnostics, however will ship all collected specimen's directly to your preferred laboratory. Simply provide us with your preferred laboratory name and account number and we will ship at no additional charge to you. We will deliver your specimen to a local lab or will ship via Fed Ex, USPS, UPS, etc.

WHEN WILL MY PROVIDER RECEIVE MY RESULTS?

Your selected preferred laboratory should send all results to the ordering provider. Processing time usually takes 2 to 3 business days. Some tests may require up to 3 weeks for all results to process. You should reach out to your provider to discuss any specialty kits testing. In the event that your provider did not receive any lab results, it is the providers responsibility to reach out to the selected preferred lab to request results to be sent. In many instances, providers are provided with an account and password where results may be retrieved offline.

I HAVE A QUESTION ABOUT MY BILLING OR INVOICE THAT I RECEIVED FROM PHLEB LIFE LLC. HOW DO I GET THAT HANDLED?

Phleb Life LLC will always try its best to accommodate and work with patients and their billing. In order to resolve any billing questions or concerns, please reach out via phone call to our office at 402-534-1186. Please do NOT send any questions or concerns regarding billing to our email address as we receive numerous physician orders daily.

DOES PHLEB LIFE LLC ACCEPT INSURANCE?

No. Phleb Life LLC is a private pay company. We do not file any claims with Medicare, Medicaid, or any other private insurances. The patient / facility that we service may choose to file a claim with Medicare, Medicaid, or any other private insurance but the full amount of the bill is patient / facility responsibility. Any amount that is not cover able with the patients insurance provider still remains the patients responsibility for services rendered.

HOW DOES PHLEB LIFE LLC RECEIVE MY PAYMENT?

Phleb Life LLC accepts all major credit cards via PayPal. Patients may opt to pay for their services prior to the collection date via our website or at the time services are rendered.

THE PATIENT RESIDES IN A NURSING HOME, ASSISTED LIVING FACILITY, RETIREMENT COMMUNITY, ETC. WHAT DO WE DO IF WE HAVE QUESTIONS OR CONCERNS?

Please direct all questions and concerns to your Nursing Home, Assisted Living, or Retirement Community Administrators. Phleb Life services many different providers, patients, and clients. If a patient resides in a facility, a contract for services would be on file and all terms are outlined in the contract.
