We use several different national clinical and specialty labs for our testing.
If we have supplied you with a specialty kit, take note of the name of the laboratory at the top of your Laboratory Requisition Slip and follow the directions below to determine how to collect your specimen. All patients have the option to go to  our location at 7805 L Street, Suite 140,Ralston Ne. 68127 OR may have their blood drawn at home or office by contacting us. We will come to your home or office by appointment only to draw your blood. Your samples will be prepared and shipped by the phlebotomist. If you have been provided with a kit for specialized laboratory testing, follow the collection procedures for at home collection. Your kit contains forms, instructions and supplies. Inside the specimen kit provided you will find specimen collection and preparation instructions, specimen collection supplies, prepaid shipping envelope, and shipping instructions. If your kit comes with a freezer pack, please place the freezer pack in the freezer for at least 12 hours so it is cold when your specimen is collected. Please be sure to fill out the patient information, insurance and payment portions of the lab slip. In many instances, the lab may require you to pay a fee prior to processing the lab. DO NOT PAY THIS WITH PHLEB LIFE LLC! On the lab requistion sheet provided, fill in either your credit card or check number and the selected lab will collect your funds prior to testing your samples. This fee is completely seperate from our fee.
